Lost my channel

How can i get back my "Slow Wagens TV" channel i'm the owner but i cant broadcasting from it. because i created with my Facebook account but i already have a ustream username. when i signed with the ustream username then i connected my account with Facebook then i missed the cannel, how can i recover it? Thanks
